With the emergence of the Web, the Laboratory of Computer Science (LCS) developed a Web-based front-end to its successful Computer Stored Ambulatory Record (COSTAR) medical record system. Several years ago we began development of an application architecture based on Web services and Extensible Markup Language (XML). We now have a Web-based framework called OnCall that supports the medical records in Internal Medicine Associates (IMA), as well as specialty records for Cardiology and Cardiac Surgery, with over 1000 users.
Specialty brands of the Oncall system include Virology, Primary Care Office Insite (PCOI), Fertility, and Cardiac Surgery. Virology Oncall supports the human immunodeficiency virus (HIV) clinical research needs of the Division of Infectious Diseases. PCOI Oncall supports the needs of all primary care clinicians at Massachusetts General Hospital (MGH). Fertility Oncall supports the needs of the Vincent Memorial in Vitro Fertilization (IVF) Unit at MGH. Cardiac Surgery Oncall, introduced in 2000, has now matured into a major system that supports all aspects of the Cardiac Surgery inpatient program.
By using industrial standards and a loosely coupled architecture for both internal and external needs, we now support systems that provide portability, shareability, and aggregation of clinical data for clinical and epidemiological needs.
